Bridges To Home is an assertive community treatment (ACT) team that transforms lives through providing intensive wrap around services. ACT is an evidence based treatment that supports the full recovery and community integration for program partners. Serving 110 individuals who entered the program either homeless or at risk of homelessness, with severe and persistent mental illness, and who were either not getting treatment or were under treated – Bridges To Home has shown phenomenal success, with a 97% reduction in homelessness and a 50% reduction of hospitalization and crisis visits.
Bridges To Home is a mental health services act collaborative program led by Rubicon, with the active partnership of Community Health for Asian Americans, Mental Health Consumer Concerns, ANKA, and Shelter Inc.
